Blue Cross and Blue Shield of Minnesota this week became the first Minnesota health insurance company to offer a direct financial incentive -- $100 -- to its members who maintain a healthy lifestyle, MedCity Media reports. But they have to meet four criteria:

Low-density lipoprotein (bad cholesterol) level at or below 130 mmol/L

Blood pressure less than or equal to 120/80

No tobacco use

Body mass index (BMI) of 25 or less
