Man identified in fatal forklift accident

The operator of a forklift who was crushed by a load of wood in a fatal accident Friday afternoon in Minneapolis has been identified.

The Hennepin County Medical Examiner's Office says Travis James Carter, 36, of Ham Lake, was killed in the workplace accident at the AA Container Sales company. He died of multiple blunt-force injuries, a medical examiner's office press release said.

Carter had gotten out of his forklift when bundles of the wood product he had been stacking tumbled from the machine and crushed him, the Star Tribune reports. The Pioneer Press reported it was a pallet of plywood.

The Occupational Safety and Health Administration (OSHA) and Minneapolis Police Department are investigating.

Nationwide each year there are roughly 85 forklift fatalities and 34,900 serious injuries, the trade journal Occupational Health & Safety magazine reports, citing OSHA data.
